Unilateral stress fractures of the femoral neck are very uncommon and bilateral involvement is even rarer. The physical therapist also ordered radiographs of the pelvis and bilateral hips, which were initially interpreted as normal . There are only two cases of bilateral stress fractures of the femoral neck occurring simultaneously following a bilateral TKA [11, 12]. Femoral neck stress fractures occur mainly in athletes, military trainees, physically hyperactive people, and patients with metabolic disorders or who are undernourished 2) . It is easy to miss initially or may be misdiagnosed.
Radiographs show no evidence of a stress fracture, an alpha angle of 45 degrees, and a lateral center edge angle of 30 degrees.

A direct fall on the hip, completion of fatigue fracture or twisting of the body over the hip are all possible causes of a fractured neck in the elderly. The distal shaft, between the distal one-third and the femoral condyles, is a common site. High-risk sites of stress fractures are locations at greatest risk of a progression to complete fracture, displacement or non-union. We report this rare case with a review of the literature. Femoral neck fractures can be traumatic or stress fractures.
Treatment of Femoral Neck Fracture . The patient was successfully healed by conservative treatment. Unilateral stress fracture of the femoral neck following TKA is a rare complication; only 21 cases have been described in English literature so far [2–10] (Table 1). They commonly occur in athletes, military recruits, older persons, or individuals with underlying metabolic disorders and very seldom in normal individuals.
